The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a Case Management visit. LPA was greeted and granted entry by staff. LPA met with Assistant Administrator (AA) Alex Blancarte and discussed the purpose of the visit. Administrator (AD) Melinda Flores was notified of LPAs presence at the facility and was available by phone to assist.

LPA collected documents in congruent with complaint control # 22-AS-20210520100831.
