The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit for the purpose of conducting a case management to amend the LIC809D page from the complaint investigation 22-AS-20250930102714. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Melinda Flores and Assistant Administrator Alex Blancarte and discussed the purpose of the visit.

LPA changed the section cited from 87469.1(a)(2) Personal Rights of Residents in all Facilities to the correct section of 87468.1(a)(2) Personal Rights of Residents in all Facilities.

An exit interview was conducted with AD and a copy of this report along with amended LIC809D and Plan of Correction clearance letters were left at the facility.
