The inspector’s account

Licensing Program Analysts (LPAs) Nancy Guillen and Hanna Gough made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPAs were greeted and granted entry by staff. LPAs met with Quality Assurance Manager (QA) Merlinda Maglaya explained the reason for the visit and Administrator (AD) Martin Lopez later joined to assist with the inspection. The facility currently has five clients all of which were at Day Program.

The facility is a two story home with three client bedrooms, two bathrooms, kitchen, dining room, living room, backyard, staff office and attached two car garage. The staff office is locked making it inaccessible to clients in care. The kitchen appears is free of debris and vermin. LPAs observed the facility has a two day perishable and seven day non-perishable food supply on hand. LPAs observed expired condiments in the fridge that staff promptly threw away after being notified. LPAs observed the sharps to be locked in a drawer located under the stove in the kitchen making them inaccessible to clients in care. LPAs observed toxins locked under the kitchen sink making them inaccessible to clients in care. LPAs observed all appliances to be operational. LPAs observed the medication to be stored and locked in a closet located by the kitchen making them inaccessible to clients in care. LPAs observed that all client bedrooms had the required components and furnishings. LPAs observed that the bathrooms had toilet paper, paper towels, and textured shower floors. LPAs tested the water to be between 107.2 to 116.6 degrees Fahrenheit in client bathrooms. LPAs observed extra clean linens located in the hall closet and in the garage for client use. LPAs observed two fire extinguishers both charged with a service date of March 9, 2026. LPAs tested the fire and carbon monoxide detectors and they were found to be operational. LPAs observed the emergency food and water supply located in the garage and in the closet in the hallway.
