The inspector’s account

Licensing Program Analysts (LPAs) Hanna Gough and Nancy Guillen made an unannounced visit to the facility to conduct the required annual inspection. LPAs were greeted and granted entry by staff. LPAs met with Quality Assurance Manager (QA) Merlinda Maglaya and discussed the purpose of the visit. The facility currently has 4 clients in care.

The facility is a one story home with four client bedrooms, one staff bedroom, two bathrooms, dining room, living room, kitchen, attached two car garage and a swimming pool in the backyard. The facility appears clean, safe, and sanitary. The staff room is locked and made inaccessible to clients in care. The kitchen appears clean and free of vermin. LPAs observed the facility has a two day perishable and seven day non-perishable food supply on hand. LPAs observed all appliances to be operational. LPAs observed the sharps to be in a locked drawer located by the stove and made inaccessible to clients in care. LPAs observed toxins and chemicals to be locked under the kitchen sink and made inaccessible to clients in care. LPAs observed the medication cabinet located in the kitchen to be locked and made inaccessible to clients in care. LPAs observed the first aid kit to have all the required components. LPAs observed the client bathroom to have toilet paper, paper towels, and a non-slip mat in the shower. LPAs tested the water to be at 102.4 and 105 degrees Fahrenheit. LPAs observed extra clean linens located in a hall closet for client use. LPAs observed client bedrooms to have all the required components and furnishings. LPAs observed fire extinguishers throughout the facility charged and with a service date of March 9. 2026. LPAs and QA tested the carbon monoxide and smoke detectors and they were found to be operational. LPAs observed the emergency food and water supply in a cabinet located in the hallway. LPAs observed activity supplies for client enjoyment.
