Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA was greeted and granted entry by staff. LPA met with House Manager (HM) Merlinda Maglaya and explained the reason for the visit. The facility currently has four clients of which all are at day program.
The facility is a one story home with four client bedrooms, one staff bedroom, one client bathroom, one staff bathroom, dining room, living room, kitchen, attached two car garage and a swimming pool in the backyard. The facility appears clean, safe, and sanitary. The staff room is locked made inaccessible to clients in care. The kitchen appears clean and sanitary. LPA observed the facility has a two day perishable and seven day non-perishable food supply on hand. LPA observed the sharps to be in a locked drawer located in the kitchen and made inaccessible to clients in care. LPA observed toxins and chemicals to be locked in the kitchen cabinet under the sink and made inaccessible to clients in care. LPA observed the medication cabinet located in the kitchen to be locked and made inaccessible to clients in care. LPA observed the client bathroom to have toilet paper, paper towels, and a non-slip mat. LPA tested the water to be at 108.1 degrees Fahrenheit located in the client bathroom by the bedrooms. LPA tested the water in the staff bathroom located by the kitchen that is sometimes used for clients to be at 110.0 degrees Fahrenheit. LPA observed extra clean linens located in a hall closet. LPA observed client bedrooms to have all the required components and furnishings. LPA observed two fire extinguishers both with a service date of March 10, 2025 located in the kitchen and in the garage. LPA tested the carbon monoxide and smoke detectors to be operational. LPA observed the emergency food and water supply in a cabinet located in the hallway. LPA observed activity supplies for client enjoyment. LPA observed a shaded seating area for client use.
