Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a Plan of Correction (POC) visit in regards to citations issued on April 15, 2026, during the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Program Administrator (PD) Jaqueline Lara and discussed the purpose of the visit.

LPA reviewed completed physicians reports and TB tests for Resident #2 and Resident #3,thus clearing citation 81069(a).

LPA observed completed first aid/CPR training for Staff #2 that expires on October 15, 2026, thus clearing citation 81075(f).

Based on records reviewed all citations given on April 15, 2026, have been fulfilled by the POC due date. An exit interview was conducted and a copy of this report along with LIC858, 859 and clearance letter were provided at the time of the visit.
