Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit to the facility for the purpose of delivering an amended version of deficiency page (LIC809-D) cited during complaint investigation and generated on January 22, 2026. LPA was greeted and granted entry and met with Clinician Britney Richardson. At 3:15 p.m., Clinical Director (CD) Tony Resendiz was contacted by phone.
LPA, CD, and Clinician Richardson discussed the amendment. An exit interview was conducted and a copy of this report was provided at the end of the visit.
