Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff and met with House Manager (HM) Robbie Kenobi. LPA informed HM the purpose of the inspection. The facility currently has five clients in care. LPA observed clients watching television and lounging in their bedrooms.
The facility is a one story home with an attached two care garage. The facility has 3 client bedrooms, 2 client bathrooms, staff bathroom, a kitchen, dining room, office, medication room, and swimming pool. The facility appears clean, safe, and sanitary. All resident bedrooms had the required components and furnishings. Facility has extra clean linens for client use located in the hall closet. Restrooms are stocked with soap, toilet paper, and paper towels. Hot water in the bathrooms measured above 149.7 degrees Fahrenheit. LPA observed the kitchen to be clean. LPA observed the sharps to be locked in a drawer located in the kitchen and inaccessible to clients in care. LPA observed that there were two fire extinguishers, one in the kitchen and one in the laundry room. Both were charged and had a service date of December 26, 2024. LPA observed the centrally stored medications were locked in a cabinet located in the medication room and is inaccessible to clients in care. LPA observed the toxins and chemicals are locked in a cabinet in the office making them inaccessible to clients in care. LPA observed the facility has an emergency food and water supply. LPA observed the backyard to have shaded seating for client use. LPA observed the swimming pool to be fully enclosed. The swimming pool is surrounded on two sides by an 8ft brick wall. The other two sides are surrounded by an iron fence. The fence is 5ft tall and the bottom of the fence is 2.25 inches off the ground. The fence has vertical bars that are separated by 3.5 inches of space. The backyard was clear and free of obstructions. Carbon monoxide detectors were tested and operational.
