Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Aliza Festin and discussed the purpose of the visit. The facility currently has four clients in care.
The facility is a one story home with four client bedrooms, three bathrooms, kitchen, living room, dining room and attached 2 car garage. The facility appears clean, safe and sanitary. All client bedrooms had the required components and furnishings. The extra linens were observed to be in the hall closet by three client bedrooms. The restrooms are stocked with toilet paper, paper towels and soap. LPA tested the water to be between 112.6 and 116.9 degrees Fahrenheit. LPA observed the kitchen to be clean and free of vermin. The sharps were observed to be in a locked box under the kitchen sink and made inaccessible to clients in care. LPA observed a two day perishable and seven day non perishable food supply on hand. LPA observed a fire extinguisher charged and wish a service date of February 6, 2026 by the kitchen sink. LPA observed the emergency food and water to be stored in the closet by the front door outside the kitchen. The centrally stored medication is stored in a locked cabinet located in the dining room and made inaccessible to clients in care. LPA observed the toxins and chemicals to be stored in locked closets in the garage and made inaccessible to clients in care. The garage was observed to be used as facility storage and had an operational fridge and freezer with extra facility food. The backyard has a shaded seating area for client use and no debris or obstructions were observed. AD tested the carbon monoxide and smoke detectors and they were found to be operational.
