Facility Evaluation Report
On today's date Licensing Program Analyst (LPA) William Vanegas conducted an unannounced visit for the purposes of completing an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by facility staff. LPA advised of the purpose for the visit and Assistant Administrator (AAD) was advised. LPA Vanegas toured the facility accompanied by AAD and observed the following AAD Aliza Festin has a valid administrator certificate valid from March 13, 2025 through March 12, 2027.
This is a one storied home with four bedrooms all of which are for clients in care, two bathrooms all of which are for clients in care, one of which is a private bathroom and one of which is a shared bathroom, and a detached two car garage. LPA observed kitchen area to be clean and free of any mildew or debris. LPA observed kitchen area to have a gas stove, dishwasher, microwave, and refrigerator. All appeared to be in good repair and tested operational. LPA observed a two day supply of perishable food and a seven day supply of non-perishable food on hand, as well as a sufficient amount of water in case of a disaster.
LPA observed resident bedrooms to be free of any hazards, and debris. LPA observed bedrooms to have all required furnishings such as a bed, clean linens in good repair; meaning no strains or tares, a reading lamp, a chair,a chest of drawers and enough storage space to store personal belongings.
LPA observed resident bathrooms to be clean and free of any mildew and debris. LPA observed all water faucets and toilets to be operational. LPA observed bathrooms to have all required furnishings such as grab bars, a shower chair, and slip resistant floor matts. Hot water temperature tested between 109.5 and 110.1 degrees Fahrenheit.
