Facility Evaluation Report

Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. Upon arrival, the front gate of the facility was locked. LPA contacted Chief Strategy Officer (CSO) Reese Perez by phone and the purpose of the inspection was discussed. Per CSO, the facility is pending vendorization by Regional Center of Orange County (RCOC) and therefore, currently has a zero census. CSO stated there was currently one staff present on-site and stated they would inform them of LPA’s visit. LPA was then greeted and granted entry by Staff Jesus Lopez.

During the inspection, LPA and Staff Lopez conducted a tour of the facility and observed the following:

This is a two-story building, with one computer lab, two bathrooms, four activity rooms, two staff offices, and a main activity room on the first floor. The second floor consists of six activity rooms, a library, two bathrooms, and a staff lounge. There is no backyard or patio space. LPA observed utilities to be operational, including water and electricity. Bathrooms were observed to be free of debris and mildew and faucets and toilets were operational. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. Sharps, all and any toxic chemicals, cleaning solutions, and disinfectants are inaccessible. Fire extinguishers were observed to be fully charged with service tags dated September 2, 2025. Smoke detectors and carbon monoxide detectors tested operational. Per CSO, they wish to remain licensed at this time and will continue to pursue RCOC vendorization.

Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was left at the facility.
