Facility Evaluation Report

Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA was greeted and granted entry by Chief Strategy Officer (CSO) Reese Perez and the purpose of the inspection was discussed. Per CSO, the facility is pending vendorization by Regional Center of Orange County and therefore, currently has a zero census.

During the inspection LPA and CSO conducted a tour of the facility and observed the following:

This is a two-story building, with one computer lab, two bathrooms, four activity rooms, two staff offices, and a main activity room on the first floor. The second floor consists of six activity rooms, a library, two bathrooms, and a staff lounge. There is no backyard or patio space. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested at 103.1 F degrees.

LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. Sharps, all and any toxic chemicals, cleaning solutions, and disinfectants are inaccessible. Fire extinguishers were observed to be fully charged with service tags dated September 12, 2024. Smoke detectors and carbon monoxide detectors tested operational.

LPA conducted file review for one of one staff present and file was observed to contain all required documentation. CSO stated they will notify Community Care Licensing if, and when, they admit a client into the facility, or if there are any changes to the license.

Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was left at the facility.
