Facility Evaluation Report

LPA Hiratsuka conducted this unannounced annual visit. LPA toured the facility with Floor Manager Christina Gaughan. Assistant Director Stefanie Bond arrived during this visit.

This program is in a church. There is a great room that has a kitchen that is enclosed to keep the clients out. The kitchen has a window that opens to the great room to serve food. There is a men's and women's bathroom in a hallway behind the kitchen to the right. There is a quiet room to the left in a hallway behind the kitchen where residents can stay as well. In the great room there is a small office area that has a locked cabinet for medications if residents require medications.
