Facility Evaluation Report

LPA attempted a visit to the day program. LPA met the program social worker leaving the facility. She has been covering for the Director who is out after surgery. The social worker had an IPP meeting with Alta CA Regional Center scheduled, so had to leave. Therefore, LPA looked around quickly inside and decided to continue the annual to another day. There were no clients present. The program was closed today, and a cleaning service was shampooing the carpets.

LPA will return at a later date to finish the visit.
