Facility Evaluation Report

LPA Parks arrived on Tuesday November 29, 2022 to conduct the annual inspection. Prior to the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 mask.

LPA and Administrator completed the infection control domain together and facility was found to be in substantial compliance at this time. LPA and Administrator toured facility together to ensure health and safety of clients in care.

LPA requested updated copy of LIC500 for facility file.

No deficiencies are being cited as a result of todays inspection. Exit interview conducted.
