On 10/06/2025, at approximately 4:15 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a case management - incident visit and met with Theresa Jones, Administrator.
LPA is conducting a case management visit to obtain more information regarding an incident report and SOC341 received by the Department on 10/02/2025 for an incident that occurred on 10/01/2025, involving Resident 1 (R1) and Resident 2 (R2). LPA conducted an interview and obtained documents.
No deficiencies were cited during todays visit.
Exit interview was conducted with Administrator, whose signature on form confirms receipt of documents.
