Facility Evaluation Report
At approximately 2:00 PM, Licensing Program Analyst, (LPA) Julie Florio arrived unannounced for the purpose of conducting a facility closure inspection. LPA was greeted by Patricia Gray, Executive Director (ED). Facility operated as an Adult Residential Facility (ARF) and was a forensic re-entry program. The purpose of this inspection is to confirm the licensed program's services have been discontinued and verify that there are no clients in care. The last day of operation was April 30, 2025. There are no clients present in the facility today. There were no other staff present during today's inspection. There were two (2) clients at the time the facility announced it's closure. Both were assisted with placement elsewhere.
At approximately 2:10 PM, LPA conducted a walk-through of the facility with ED, inspected all rooms and the exterior of the building. LPA observed the facility furnished and staged. LPA was informed the facility is currently for sale. ED informed LPA that they no loner wish to operate a licensed facility due to a loss of funding. ED agreed to email the Department a statement of the facility's intent to close and a copy of the eviction letter that was sent to clients back in April 2025.
Closure inspection of this facility is complete. Facility no longer has their license, so no license was surrendered during inspection. The Department will move forward with the closure process. LPA to finalize paperwork for closure.
No deficiencies cited during today's visit.
