Facility Evaluation Report

An office meeting was conducted today, 02/18/2026, in the Santa Rosa Regional Office. The following individuals were present in the meeting: Licensing Program Manager, Bethany Moellers, Licensing Program Analyst, Julie Florio, Facility Program Director, Emmy Bennett, and Facility Executive Director, Jamie Thompson. The purpose of the informal office meeting was to discuss one incident which occurred 07/02/2025 at the organization's Brown Street location in Napa, CA and two other incidents which occurred on 10/01/2025 and 01/21/2026 at the organization's Imperial Way location in Napa, CA. This is to follow up regarding concerns the Department has surrounding these incidents.

Items addressed in today's meeting include but are not limited to the areas below :

Incidents of alleged abuse of clients in care by staff

Alleged staff on client incident which occurred on 07/02/2025 at the Brown Street location and resulted in the facility conducting an internal investigation which ultimately led to the staff member being terminated.
