Facility Evaluation Report
At approximately 3:45 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and met with Minerva Villegas, Administrator. Facility is a Residential Care Facility for the Elderly (RCFE) with 28 residents in care and serves assisted living and memory care residents. Community has a Hospice waiver for 12.
LPA obtained a copy of the resident roster, facility census, and staff roster. LPA will return at a later date to complete this annual inspection.
No Deficiencies are being cited during today's inspection.
Exit interview conducted with Administrator whose signature on form confirms receipt.
