Facility Evaluation Report
At approximately 1:30 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and met with Cheree Truso, Program Director (PD). Facility is a Social Rehabilitation Facility (SRF) with eleven (11) clients in care.
At approximately 2:15 PM, LPA initiated a tour of the facility with PD and observed the following: Facility is a one story home, was a comfortable temperature, and passageways were free from obstructions. Water temperatures in client bathrooms measured within the allowable range of 105 to 120 degrees F or had a hot water warning was posted above the faucet per Title 22 regulations. LPA observed a supply of clean linens, hygiene, and paper products available for clients. Client bedrooms were inspected and observed to have all of the appropriate furnishings per regulation. Facility has at least two days of perishable food and one week of non-perishable foods for emergency preparedness. Medications were centrally stored and locked. There is a shaded seating area in the backyard with outdoor space for activities. LPA observed a locked structure on the property which contained mostly tools, chemicals, supplies, equipment, and holiday decorations. Facility has internet access and an internet access device available for client use. Facility's telephone was tested and operational during inspection.
Facility's fire extinguishers were observed charged and were last inspected 01/2026. Facility has a sprinkler system which is inspected yearly by a third party vendor and was last inspected 07/2025. The local fire department inspects the facility yearly and was last inspected 08/2025 where all smoke and carbon monoxide detectors were operational.
