Facility Evaluation Report

At approximately 11:00 AM, Licensing Program Analyst (LPA) Julie Florio arrived at this facility unannounced to conduct a Case Management Legal/Non-Compliance visit and was greeted by staff. Norbert Sacro, Licensee was contacted via telephone and arrived at approximately 11:30 AM. This facility was placed into Non-Compliance for the following reasons:

Failure to seek timely medical care for a resident's worsening wound; and

Failure to ensure resident received ongoing medical care for a worsening wound, resulting in resident's hospitalization.

LPA toured the facility to ensure residents were safe, secure and that their medical needs are being met.

No citations issued during today's visit.
