12/15/2024 and conducted by Evaluator Christopher Arnhold

COMPLAINT CONTROL NUMBER: 21-AS-20241215231016

Staff did not properly notify of alterations to the facility grounds

At approximately 11:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct an investigation into the above allegation. LPA met with House Manager Rowena Radana and Ed David. Based on interviews conducted, this facility has been licensed since 05/17/2023 and no alterations have been made to the facility since that time.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.
