Facility Evaluation Report
At approximately 10:00 AM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection, quarterly non-compliance visit, and conduct a Plan of Correction (POC) visit. LPA met with House Manager, Ed David. Facility is an Adult Residential Facility (ARF) with three (3) clients in care. All clients were at day program during today's visit. Facility is vendored with North Bay Regional Center (NBRC).
LPA observed the Administrator was not and had not been present at the facility. On 12/30/2024, LPA Chris Arnhold conducted an unannounced visit to verify administrator presence at the facility and cited and requested a written statement from Licensee and an updated LIC500, Personnel Summary. The documents were to be submitted by POC date of 01/06/2025. LPA Arnhold conducted a subsequent unannounced visit on 01/15/2025 and issued a civil penalty of $100 per day, for nine (9) days, totaling $900, for failing to correct the deficiency. LPA Arnhold conducted a second subsequent unannounced visit on 02/19/2025 and issued additional civil penalties of $100 per day, for thirty five (35) days, totaling $3,500. As of today's date, none of the requested documents have been received by the Department. A civil penalty of $100 per day, for eighty four (84) days is being issued, totaling $8,400. A civil penalty of $100 per day will continue to be assessed until the requested information has been received by the Department. LPA attempted to contact Licensee/Administrator via telephone and was unsuccessful. LPA was informed that Licensee/Administrator sold the business and property to two separate parties in April 2025.
