Facility Evaluation Report

An office meeting was conducted today, 11/19/2025, in the Santa Rosa Regional Office. The following individuals were present in the meeting: Licensing Program Manager, Bethany Moellers, Licensing Program Analyst, Julie Florio, Licensee, Jean Felix and prospective new Licensee, Kristine Bernardino and spouse Ron Hael. The purpose of the informal office meeting was to discuss the current operation of the facility and continued financial monitoring compliance. This is part of the non-compliance plan facility was placed on 08/19/2024 during a formal office meeting regarding concerns the Department has surrounding the facility's financial solvency.
