Facility Evaluation Report
At approximately 5:30 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and met with Kamille Nastor, Designated Responsible Party (DRP). Facility is an Adult Residential Facility (ARF) with four (4) clients in care. All clients were present during the inspection. Facility is vendorized with North Bay Regional Center (NBRC).
LPA obtained documents and will return at a later date to conduct the full annual inspection including file review.
No deficiencies cited during today's inspection.
Exit interview conducted with DRP whose signature on for confirms receipt of documents.
