Facility Evaluation Report
At approximately 1:00 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection. Initially, no one answered the facility door. LPA contacted Licensee who contacted staff at the facility. At approximately 1:20, LPA was greeted by Kamille Joyce Nastor, Designated Responsible Party (DRP). Facility is an Adult Residential Facility (ARF) with three (3) clients in care. All clients were away at their day programs during inpsection. Facility is vendorized with North Bay Regional Center (NBRC).
At approximately 1:45 PM, LPA initiated a tour of the facility with DRP and observed the following: Facility is a one story home, was a comfortable temperature, and passageways were free from obstructions. Water temperature in clients' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens and paper products available to clients. Clients' bedrooms were inspected and observed to have all of the appropriate furnishings as outlined in Title 22 regulations. LPA observed a broken toilet seat and toilet tank lid in client bathroom two (2) as well as water damage under the kitchen sink, (see LIC809D). Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were observed centrally stored and locked. LPA observed an unsecured fireplace. DRP covered it immediately and agreed to operate in compliance with regulation moving forward. LPA observed structural changes in the facility garage area where a newly constructed employee lounge and two bedrooms were added to the facility and not accurately reflected on the current facility sketch or fire clearance.
