Facility Evaluation Report

At approximately 1:40 PM, Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a Case Management - Other visit and met with Housemanager Marcia Torres.

The reason for this visit is follow up on a deficiency cited during annual inspection on 5/7/2026. Facility was cited following a file review that showed one (1) of two (2) clients did not have a current Appraisal Needs and Service Plan. During todays visit LPA reviewed client file and found that facility has an updated Functional Behavior Assessment/Behavior Intervention Plan dated 6/26. The deficiency cited on 5/7/2026 has been cleared during this visit.

No deficiencies cited. Exit interview conducted with Housemanager, whose signature on form confirms receipt.
