Facility Evaluation Report
Licensing Program Analysts (LPAs) Arnhold and Mutialu arrived unannounced to conduct an Annual Required inspection and was greeted by House Manager, Marcia Torres, Gabriela Echevarria, Administrator arrived shortly after.
LPAs initiated a tour of the facility around 9:00 A.M. and made the following observations: Facility was a comfortable temperature ranging from 71 and 74 degrees F. Resident rooms were furnished per regulation. Water temperature in bathrooms used by residents measured at 111 degrees F which are within the range of 105 to 120 degrees F allowed per regulation. Extra hygiene products and linens were available. Cabinets containing cleaning supplies were inaccessible. Client's medications were centrally stored and locked. LPA's observed staff medication in the kitchen and garage unsecured. LPAs observed perishable foods uncovered, not properly stored, or labeled with date. LPAs observed scissors and knives stored in an unlocked drawer. LPAs observed side fence is in need of repair, debris in the backyard, cob webs throughout the facility, and couch and chairs in need of replacement. One out of one fire extinguishers were last serviced March 2024. Three out of three smoke detectors and one out of one Carbon Monoxide detectors located throughout the facility were tested and operational during inspection. Facility did have their most recent fire/disaster drill recorded and are up to date as of April 2024. Facility has Personal Protective supplies.
At approximately 10:30AM, Two staff files and two resident files were reviewed. Staff files reviewed contained the required documentation, including First Aid certification. Administrator Certificate for Administrator, Gabriela Echevarria is expired. Administrator informed LPA's all the renewal paperwork was submitted on time. Medications and medication records were reviewed. LPAs observed centrally stored medication records are not up to date.
