This is an official report of an unannounced visit/investigation of a complaint received in our office on

01/08/2024 and conducted by Evaluator Christopher Arnhold

COMPLAINT CONTROL NUMBER: 21-AS-20240108094519

At approximately 9:30AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct an investigation into the above allegation. LPA met with Administrator Kam Gantan, Applicant Babita Dhawan and reviewed records. LPA reviewed application documents submitted by applicant. LPA observed Kam Gantan is not listed on the application as anything but Administrator. The applicant, Serene Care Home LLC, is the current management company put into place in 2023 during a court ordered receivership. Based on interviews conducted with Administrator and Applicant, the physical property was sold to Boda Properties LLC during the receivership in 2023. Kam Gantan has no financial involvement with Boda Properties or Serene Care Home LLC. At this time, the application is still pending and there has not been a transfer of the License.

This agency has investigated the above allegation. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis.
