Facility Evaluation Report
At approximately 10:00 AM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and was greeted by staff. Licensee was contacted via telephone and arrived at approximately 10:45 AM. Facility is an Adult Residential Facility with six (6) clients and one (1) Tenant currently living there. Facility is licensed for six (6) ambulatory clients. LPA was informed that three (3) clients were away at their day programs.
At approximately 11:15 AM, LPA initiated a tour of the facility with Licensee and observed the following: Facility is a one-story home, was a comfortable temperature, and passageways were free from obstructions. Water temperature in clients' bathrooms measured within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens available to clients. Clients' bedrooms were inspected and had all of the required furnishings as required in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPA observed a supply of paper products available to clients. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were observed centrally stored and locked.
There is a shaded seating area and outdoor space for activities in the backyard. LPA observed a locked shed in the backyard which contained personal items, furniture, equipment, and chemicals. Facility has internet service and Licensee agrees to purchase a designated internet access device and have it available to clients in care. Facility phone was tested and operational during today's inspection.
Facility's fire extinguisher was observed charged and was last inspected 7/2024. Licensee agrees to have it serviced to bring the facility back in to compliance with regulation.
