Facility Evaluation Report

On 10/21/2025, at approximately 10:00 AM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a case management - other visit and met with Rose Salgado, Licensee.

LPA is conducting a case management visit to amend complaint control number 21-AS-20250618153003 substantiated complaint finding which was delivered on 09/29/2025. LPA is issuing an immediate civil penalty in conjunction with the amended complaint finding per Title 22 Regulations.

During today's visit, LPA also discussed two (2) newly constructed half walls permanently secured to the floor which were observed in the room identified on the facility sketch as "living room 2" off of the dining area. Licensee agrees to have the local fire department inspect them for approval within thirty (30) days of this visit. LPA informed Licensee that this area shall not be set up as a bedroom for staff or resident use. Licensee conveyed understanding and agreed to ensure the facility operates in compliance with regulation.

No deficiencies were cited during todays visit.

Exit interview was conducted with Licensee, whose signature on form confirms receipt of documents.
