Facility Evaluation Report
Licensing Program Analyst (LPA) Araceli Canela arrived unannounced to conduct an annual Required-1 year inspection and met with Program Director, Brittanni Murphy. Administrator Maxime Konan was not present during this visit. The annual inspection is focused on the Infection Control procedures and practices of this Adult Day Program.
This Adult Day Program is currently providing in-person services on Monday- Friday 9am to 3pm. There is currently 10 participants enrolled in the program and 7 were present during todays inspection. Fire Extinguishers were found to be last charged on June 1, 2021.
Upon arrival, LPA came in through another entrance to the facility, Program director explained everyone comes in through another main entrance and they do the screening and sign in at that door. Staff took LPAs temperature and went over screening questions. Infection control practices are present: entry procedures, face coverings, daily monitoring and temperatures checked for participants, staff, and 30-day PPE supply. LPA conducted a walk-through of the facility with Program Director and observed COVID-19 precaution postings. Adult Day Program to follow indoor visitation requirement of verifying and tracking COVID-19 vaccination or verify non-essential visitors have proof of a negative COVID-19 test. Staff clean and disinfect the facility daily. Bathrooms are equipped with liquid soap and paper towels. Staff have completed PPE training but have not been N-95 Fit tested. This Adult Day Program has a COVID-19 Mitigation Plan Report on Epidemic Outbreaks specific to COVID-19 that was approved on 4/19/2021.

Exit interview conducted with Brittanni Murphy and a copy of this report emailed to facility due to printer problems. No deficiencies cited during today's inspection
