Facility Evaluation Report
On June 16, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to open complaints numbers 24-AS-20260616081015 & 24-AS-20260611082217 . Case management is being done on the following.
While at the facility LPA observed an unlocked closet door with cleaning supplies and toxins. LPA asked why the storage was unlocked and staff stated they had used it.
LPA was also previously informed R1 and R2 are currently in the hospital. LPA asked staff to provided verification of incident reports that were sent to the Dept, but were not able to provide verification. S1 stated the incidents were documented but reports were not sent to the Dept.
Both deficiencies were cited during today's visit.
Exit interview was conducted and a copy of this report LIC809, LIC809D, and appeal rights were provided to Administrator Emmanuel Inneh.
