On 7/14/25, Licensing Program Analysts (LPAs) M. Medina and D. Boyd conducted an unannounced Case Management visit to conclude the inspection tool and review staff files. LPAs introduced self, provided identification, and allowed entrance. Administrator, Emmanuel Inneh contacted by telephone and arrived a short time later to conduct visit with LPAs.
LPAs met direct care staff to conduct facility tour. LPA Medina reviewed a sample of staff files and observed the files to have the required documentation and staff training's.
LPA Boyd will document the physical plant tour and inspection tool results on a separate report.
