Facility Evaluation Report
On 6/19/25 Licensing Program Analyst (LPA) Daiquiri Boyd arrived at the facility unannounced to conduct an annual inspection. LPA was greeted and allowed entry into the facility. Administrator- Emmanuel Inneh was contacted via phone and was not able to attend the inspection due to being out of the country for a family funeral. Staff Larry Rimas conducted the tour with the LPA and will be signing the report.
Couches are clean and in good condition. House has tile floor throughout, which is clean, but is broken in many spots.
Caregiver Rimas stated that the fire department has come and cleared the smoke detectors, but was unable to give documentation.
Carbon monoxide readers were in working order. Areas of the facility needs to be cleaned. LPA observed kitchen to be cluttered unclean. Cabinets where clean pans and dishes are stored have debris & buildup.
LPA was unable to complete inspection due to appointment of Caregiver that could not be rescheduled
