The inspector’s account

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Mila Green, Continual Administrator's Certification expired 12/21/2023 is listed as pending on the Social Services website. There are currently 4 residents currently living in this home. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Licensee toured facility upstairs bedroom with cell phone via face time as Resident was not allowing in bedroom or upstairs area of facility.

LPA Hurt reviewed facility staff, and resident files on visit 11/09/2023.

LPA Hurt observed food inside refrigerator with no labels or dates. LPA Hurt observed kitchen bar stool furniture ripped, and facility side fence is in disrepair. Toxins and cleaning supplies are in an area in the garage open and accessible to residents.

The following deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22.
