Facility Evaluation Report

Licensing Program Analyst (LPA) Sarah Hurt arrived at the facility on 11/06/2024 to conduct this facilities Annual Inspection/ collect the facilities license.

LPA Hurt spoke with Program Director Terry Rhodes who has notified Licensing the program is now only community based through San Andreas Regional Center. LPA Hurt spoke with San Andreas Regional Regional Resource Specialist Molly Sullivan who confirmed the program is community based and no longer requires a state license.

LPA Hurt contacted Program Director Terry Rhodes at (831)233-2777 requesting to meet at the facility to tour and collect the license. LPA Hurt left voice mail for Program Director Terry Rhodes, and is waiting for return phone call.

LPA Hurt will return at a later date to collect facility License.
