Licensing Program Analyst (LPA) Sarah Hurt conducted a Plan of Correction visit. LPA met with facility Facility Administrator, Jose Vitan and explained the purpose of today's visit.
LPA observed the facility ceiling in room 12 is repaired.
LPA tested the water temperature in resident bedrooms 2 and 3, and between 7 and 8, and also in the men's room. The water temperatures all measured between 105 and 120.
Administrator emailed on 11/21/2025 requesting an extension for the remaining deficiencies until 12/15/2025.
Exit interview conducted with Administrator Jose Vitan, and a copy of this report along with appeals rights provided.
