Facility Evaluation Report

Licensing Program Analysts (LPA) Sarah Hurt and Shawna Doucette conducted an unannounced visit today for the facility’s annual inspection (continuation). LPA met with facility staff, Jose Vitan.

Resident 4's medical assessment documents they should not leave the facility unassisted. Resident 4 frequently leaves the facility unassisted walking as far as half mile from the facility on their own. Resident 5's Admission agreement does not list amount to be paid to the facility monthly. LPA's observed the facility does not have all staff records available for review as required. Facility staff 4 does not have required completed Health Screening.

The following deficiencies are being cited per Title 22 Regulations. Exit interview conducted with Facility staff, Jose Vitan, and a copy of this report along with appeals rights provided.
