Facility Evaluation Report
Licensing Program Analysts (LPA) Sarah Hurt and Shawna Doucette conducted an unannounced visit today for the facility’s annual inspection. LPA met with facility staff, Jose Vitan, and Richard Macasaet Continual Administrator's Certification expires 05/16/2026. There are currently 26 residents who reside at this home and there is 0 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, medication storage, kitchen, and outdoor areas. There is a locked storage for medications.
Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible.
LPA's observed several cans of expired food in pantry area. LPA's observed the area around food storage has webs and appears dirty. LPA's observed food stored inside dirty containers. The facility does not have required food supply. The facility does not have a weekly food menu. Resident 1 and Resident 2 were both treated for lice. The facility did not report the lice to Licensing.
