Licensing Program Analyst (LPA) Sarah Hurt arrived to the facility unannounced to conduct a Case Management visit, and met with Facility staff, Richard Macasaet.
Resident 1's Needs and Services plan is not current / updated annually as required.
Resident 1's medical assessment is not current/updated annually as required.
Resident 1 had multiple missed medications in December 2025 and March 2026.
Resident 1 was taken to the hospital by ambulance on 04/05/2026. The facility did not report this incident to Licensing.
The following deficiencies are being cited Per Title 22 Regulations.
