Facility Evaluation Report

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Case Management. LPA met with Facility Assistant Administrator, Richard Macasaet, and explained the purpose of today's visit.

The facility is requesting an extension for the Plan of Correction citing 80020(a) All facilities shall secure and maintain a fire clearance approved by the city or county fire department, the district providing fire protection services, or the State Fire Marshal, dated 01/30/2026. Administrator emailed pricing details of alarm repair and has committed to the repairs. The alarm company has not provided exact date repairs will be completed. LPA will continue to follow up until alarm fire clearance is submitted.

No deficiencies cited Per title 22 regulations. Exit interview conducted with facility Facility Assistant Administrator, Richard Macasaet, and a copy of this report provided.
