The inspector’s account

Licensing Program Analyst (LPA)'s S. Hurt and S.Doucette arrived at the facility unannounced to conduct an Required Annual Inspection. LPA's were granted entry by Licensee Emmanuel Inneh.

LPA's toured the facility with Licensee. LPA's observed cockroaches throughout the facility and in the food supply pantry. LPA's observed spider webs and spiders throughout the facility. In Rooms 12, 14 and 15 the carbon monoxide was inoperable. Thermostat in facility is inoperable. LPA's were unable to determine the temperature in the facility.

In resident bathrooms, LPA's observed mold on the ceiling, on the tile floor and around the outside of the toilet. Bathroom vents were dirty with spider webs. Walls and doors throughout the facility were dirty. Facility bathroom faucet was spraying water outside of the sink and onto the floor.

In resident bedrooms LPA's observed the mattress springs to be broken in several resident rooms. Some mattresses did not fit the beds in resident rooms. Resident bedrooms did not have a dresser. Resident rooms night stands had broken drawers and were dirty. Closet doors in several resident rooms were broken. Bedroom screens were broken in several rooms. Window coverings were broken in several rooms.

Fire extinguishers were serviced 2/20/2024. LPA's observed a complete first aid kit.

Due to time constraints LPA's will return at a later date to complete Annual Inspection.
