Facility Evaluation Report

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Case Management. LPA met with facility Administrator Sara Post, and explained the purpose of today's visit.

Resident 1 was lives on the third floor and was alarmed by the fire alarm being triggered. There was no emergency or fire inside the facility. The facility staff did not come to the third floor to reassure residents there was no emergency inside the facility building.

Technical Assistance was reviewed, and exit interview conducted with facility Administrator Sara Post, and a copy of this report provided.
