Facility Evaluation Report

On 04/06/2026, Licensing Program Analyst (LPA) V Gorban arrived unannounced for an Annual Required Inspection. LPA met with Day Programs Manager (DPM) Greg Dinsmore. LPA introduced self, stated the purpose of the visit and was allowed entry. During this visit LPA toured facility with DPM inside and out. Facility has one entrance/exit point. At the time of the visit there were 28 clients present and 12 staff members.

The facility section for day program is currently one story building section on. The facility appeared clean, comfortable temperature recorded at 75 degrees Fahrenheit with no obstruction or fire clearance issues. Hand sanitizer was readily available to residents and visitors. Bathrooms have trash cans with lids. The exterior tour was conducted. The facility licensed with capacity for 90 clients. LPA reviewed clients files.

Kitchen observed with free of clutter and obstruction. The kitchen refrigerator appeared clean. All clients visiting day program bring its own food which is heated by staff before consumption.

Cleaning supplies were observed to be locked in a hallway cabinet. Fire extinguisher in compliance, service date 01/14/2026. Last fire drill recorded on 02/27/2026

Facility files reviewed; client's files were found to be complete and current. Staff records indicated staff responsible for the care of clients had current first aid/CPR certificates. Records indicates that all facility staff have received criminal record clearances and/or are associated to this facility.

No deficiencies were observed or cited during this visit.

Exit interview was conducted. Copy of this report printed and provided for facility records.
