Facility Evaluation Report

Licensing Program Analyst (LPA) Marybeth Donovan conducted an unannounced Required - 1 Year Annual Inspection to include Infection Control site visit and met with Amy Wright Director of Operations.

LPA toured the facility inside and out. All fire exit routes were free and clear of obstructions. LPA observed a supply of PPEs. Isolation Cart Prepped and Ready for use. These supplies are inventoried and restocked regularly.

Facility observed to have designated entry point for COVID 19 symptom screening and temperature check. Bathroom observed to be supplied with hygiene products and hand washing signs posted. Hand sanitizer available for residents, staff and visitors. Postings included Visitor Policy, Keep Safe Wear a Mask, How Can I Protect Myself, Symptoms of COVID 19, COVID 19 When to Seek Care, Stop the Spread, Cough Etiquette, Six Feet Apart and Donning and Doffing of PPEs.

LPA reviewed the facility policies and procedures to include screening, visitation, isolation, disinfecting, sick leave polices, training, PPE usage and N95 Fit Testing.

No citations issued per the California Code of Regulations Tittle 22.
