Facility Evaluation Report

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Plan of Correction visit. LPA met with facility Licensee Lydia Magallanes, and explained the purpose of today's visit.

LPA observed the facility is being painted. Furnishing in living area is covered. LPA will return at a later date to complete Plan of Correction visit. LPA submitted a referral in Sharepoint for Licensee to complete Technical Support training. LPA will return after Licensee completes Technical Support training.

No deficiencies cited Per Title 22 Regulations. Exit interview conducted with facility Licensee Lydia Magallanes, and a copy of this report provided.
