Facility Evaluation Report
Licensing Program Analyst (LPA) Sarah Hurt arrived unannounced to conduct a Case Management visit on 05/07/2025 at 03:00 p.m. LPA met with facility Assistant Program Director Itzel Pantoja, and stated the purpose of the visit.
On 04/15/2025, Resident 1 was given two doses of night time medication by facility staff. Staff 1 gave Resident 1 a dose of medication approximately 5 p.m., and staff 2 did not review the medication page documenting medication was already given and gave Resident 1 a second dose of the same medication later in the evening. This incident was self reported by the facility to Licensing.
The following deficiencies are being cited Per title 22 Regulations.
Exit interview conducted with Facility Assistant Program Director Itzel Pantoja, and a copy of this report along with appeals rights was left at the facility.
