Facility Evaluation Report

Licensing Program Analyst (LPA) V Gorban conducted an unannounced visit today for the facility’s annual inspection. LPA met with Christina Castro explained the purpose of today's visit and was allowed entry. There is currently 12 clients receiving services at this location. This facility does not have a locked storage for medications as no clients take medications.

Fire extinguisher is within the safety regulation period. Per records review disaster drills completed monthly, last drill recorded on 07/23/2026. Smoke alarms were tested and are operational. The facility has a carbon monoxide detector and observed operational.

First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible.

LPA reviewed 5 client files, and 5 staff files. LPA reviewed facility Emergency Disaster plan, Infection Control Plan, and Program Design.

Facility Records were reviewed including Plan of Operation, Emergency Disaster Plan, and Infection Control Plan.

There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22.
