Facility Evaluation Report
On 08/07/2025 Licensing Program Analyst (LPA) V Gorban arrived unannounced for an Annual Required Inspection. LPA met with Senior Director, Tiffany Dorado. LPA stated the purpose of the visit.
A tour of the facility was conducted. Visitor log-in/check was observed upon entry. Facility has one entrance/exit point. At the time of the visit there were 10 clients present. During the day, clients attend out of town events on daily bases and just returned to the facility from outing.
LPA toured the facility with Facility appeared clean with no obstruction or fire clearance issues. Fire extinguisher service date recorded on July 21/2025. Fire and carbon monoxide detectors appear operational. Recent disaster drill recorded on July 25, 2025.
LPA toured the facility rooms and observed each room is fully furnished.
Cleaning supplies were checked, locked in a closet for staff use only.
LPA reviewed clients records ensuring facility following title 22 requirements.
Facility Records were reviewed including clients files, staff files, Plan of Operation, Emergency Disaster Plan, and Infection Control Plan.
Exit interview was conducted. No citations were issued on this visit. Copy of this report printed and provided to staff for facility records.
